對於常染色體隱性遺傳疾病而言,受影響的孩子通常是從雙方遺傳來源各自遺傳到同一基因中的致病變異。如果卵子捐贈者與精子提供者都是同一種常染色體隱性遺傳疾病的帶因者,每次懷孕通常有25% 的機率孩子受到該疾病影響、50% 的機率孩子成為帶因者,以及 25% 的機率沒有遺傳到雙方的家族變異 

 

因此,如果卵子捐贈者被發現帶有某項基因變異,並不代表一定需要將她排除。ASRM 指出,如果捐贈者是某種常染色體隱性遺傳疾病的異型合子帶因者,而該帶因狀態本身不會對捐贈者的健康造成風險,則不一定需要因此排除她。相反地,應將她的結果與精子提供者的遺傳篩檢結果一併評估。 

X 染色體連鎖遺傳疾病的情況則有所不同。 由於相關基因位於 X 染色體上,其生育及遺傳風險會根據特定疾病以及胚胎所遺傳的性染色體而有所不同。ASRM 一般建議排除帶有 X 染色體連鎖疾病的卵子捐贈者,但也承認某些特定疾病可能存在例外情況。

因此,一般的卵子捐贈者遺傳篩檢流程可能包括: 

個人及家族病史 → 遺傳諮詢/風險評估 → 遺傳帶因篩檢 → 檢視檢測結果 → 與精子提供者結果進行比對 → 必要時進一步檢測 → 醫療許可 

 

ASRM 也建議,在可能的情況下,應取得卵子捐贈者詳細的三代家族病史(Three-Generation Family History)。如果家族病史顯示可能存在某種遺傳性疾病,則可能需要由合格的遺傳專業人員進一步評估,並考慮進行其他相關檢測。

例如,假設一位卵子捐贈者被確認為某種常染色體隱性遺傳疾病的帶因者。 

 

如果精子提供者對相關疾病的檢測結果為陰性,生下受該疾病影響孩子的風險通常會大幅降低,但基因檢測並不能將風險完全降至零。這種仍然存在的可能性稱為殘餘風險(Residual Risk)

 

如果雙方遺傳來源都是同一種常染色體隱性遺傳疾病的帶因者,準父母應接受遺傳諮詢,以充分了解潛在風險以及可考慮的生育選項。根據特定疾病及醫療情況,這些選項可能包括選擇另一位卵子捐贈者,或考慮針對特定疾病進行胚胎著床前單基因遺傳檢測(PGT-M)

捐贈卵子的 PGT-A 檢測:成功率與染色體準確性

對於使用捐贈卵子的準父母而言,胚胎著床前染色體非整倍體檢測(Preimplantation Genetic Testing for Aneuploidy,PGT-A)可以在胚胎植入前,提供更多關於胚胎染色體狀態的資訊。PGT-A 主要篩檢胚胎是否存在染色體非整倍體(aneuploidy),也就是染色體數量異常的情況,並可協助胚胎學家辨識被分類為染色體正常(euploid)、染色體異常(aneuploid),或在某些情況下屬於嵌合型(mosaic)的胚胎。 

 

然而,在評估PGT-A 檢測、捐贈卵子與成功率之間的關係時,需要謹慎解讀相關數據。由於卵子捐贈者通常較年輕,並且已接受相關醫療篩檢,因此使用捐贈卵子形成的胚胎,本身通常具有相對較高的染色體正常率(euploidy rate)。 

 

目前的研究證據尚未證實對所有捐贈卵子形成的胚胎常規進行 PGT-A,能夠提高活產率。美國生殖醫學會(ASRM)指出,整體現有證據並不支持在所有捐贈卵子療程中常規使用 PGT-A。 

PGT-A 如何應用於捐贈卵子形成的胚胎

PGT-A 是在捐贈卵子完成取卵,並透過 IVF 與精子受精之後進行的。PGT-A 並不是直接檢測卵子本身,而是分析受精後形成的胚胎細胞。

 

整個流程通常如下: 

捐贈者取卵 → 受精 → 胚胎發育 → 囊胚 → 滋養外胚層細胞切片 → PGT-A 分析 → 胚胎冷凍 → 取得檢測結果 → 選擇胚胎 → 胚胎植入

 

當胚胎發育至**囊胚階段(Blastocyst Stage)**時,通常約為胚胎發育的第 5–7 天,胚胎學家會從滋養外胚層(Trophectoderm)中小心取得數個細胞。滋養外胚層是囊胚中未來主要發育形成胎盤的部分。 

 

取得細胞樣本後,胚胎通常會進行玻璃化冷凍(Vitrification),而切片取得的細胞則會送往遺傳檢測實驗室進行分析。 

 

現代 PGT-A 檢測平台主要評估胚胎染色體的拷貝數。根據檢測結果,胚胎可能被分類為: 

  • 染色體正常(Euploid): 受檢測的細胞顯示預期的染色體數目。  
  • 染色體非整倍體(Aneuploid): 受檢測的細胞顯示一個或多個染色體數量異常。  
  • 嵌合型(Mosaic): 檢測樣本中顯示具有不同染色體結果的細胞混合情況。 

這些資訊可以幫助醫師在選擇胚胎進行植入時提供更多參考,但需要了解的是,PGT-A 屬於篩檢工具,並不能保證胚胎的染色體或整體遺傳健康完全正常 

 

PGT-A 所分析的是從滋養外胚層取得的少量細胞,而不是檢測胚胎中的每一個細胞,因此仍需要考量技術與生物學上的限制,包括胚胎嵌合現象(mosaicism) 

 

此外,PGT-A 也不應與**遺傳帶因篩檢(Genetic Carrier Screening)或 PGT-M(胚胎著床前單基因遺傳檢測)**混淆。 

 

PGT-A 主要評估的是胚胎染色體數目是否異常,並不能篩檢所有可能的遺傳性疾病 

PGT-A 對臨床懷孕率與活產率的統計影響

當準父母搜尋**捐贈卵子 PGT-A 檢測成功率(PGT-A testing donor eggs success rate)**時,很容易認為選擇經過染色體篩檢的胚胎,就能自動提高懷孕或活產的機率。然而,目前的研究證據並不支持如此簡單的結論。

 

ASRM 引用的一項捐贈卵子研究比較了接受 PGT-A 與未接受 PGT-A 的療程,結果顯示,使用 PGT-A 的持續懷孕/活產率為 64.4%,未使用 PGT-A 則為 54%。儘管數據上存在差異,但該差異並未達到統計學顯著性(not statistically significant),而且該研究所包含的療程數量相對較少。 

 

另一項規模較大的配對研究針對使用玻璃化冷凍捐贈卵子的療程進行分析,結果如下

 在這兩項活產率的比較中,差異皆未達到統計學顯著性。研究人員因此認為,在這些使用捐贈卵子的受贈者療程中,PGT-A 並沒有顯著提高活產的機率。 

 

另一項規模更大的分析則研究了 18,562 個捐贈卵子 IVF 療程,其中包括 11,348 個使用新鮮捐贈卵子的療程,以及 7,214 個使用冷凍解凍捐贈卵子的療程

 

在有囊胚可供使用的新鮮捐贈卵子療程中: 

  • 使用 PGT-A 的首次胚胎植入活產率為 46.9%,而未使用 PGT-A 則為 53.2%。  
  • 使用 PGT-A 的累積活產率為 58.4%,而未使用 PGT-A 則為 66.6%。
